How can I get evaluated?

Along with self-referrals, we accept referrals from private and group practices, home health and case management agencies, insurance carrier referrals, hospitals, assisted living facilities, nursing homes, rehabilitation programs & facilities, and foster care homes.

You will be sent clinical paperwork to complete and an appointment will be setup for an evaluation, either via telehealth or home visit.

Do you take insurance?

In addition to Medicare and Medicaid, we accept many major carriers in the state of Hawaii. Please contact us to inquire further.
